p-tau217

Phosphorylated Tau at Threonine 217 (Plasma / CSF p-tau217)

Highly sensitive fluid biomarker reflecting both cerebral amyloid-beta plaque burden and early tau tangle phosphorylation in Alzheimer's disease.
Target CNS Conditions:Alzheimer's & DementiaFrontotemporal Dementia

1. Biological & Technical Mechanism

Phosphorylated tau at threonine-217 (p-tau217) is a soluble tau isoform released into cerebrospinal fluid (CSF) and blood plasma in response to cerebral amyloid-beta (Aβ) pathology and early neurofibrillary tangle formation. Compared with earlier markers such as p-tau181, plasma p-tau217 exhibits markedly larger fold-changes between amyloid-positive and amyloid-negative individuals, achieving diagnostic accuracy comparable to amyloid PET and CSF assays.

(Japanese Reference):

2. Open Science & Clinical Trial Significance

Plasma p-tau217 is transforming clinical trial recruitment (pre-screening prior to PET scans) and longitudinal treatment monitoring in Alzheimer's pipelines tracked on Alzforum and ClinicalTrials.gov.

(Japanese Context):

Development and External Validation of a Plasma p-tau217-Based Model for MCI-to-Dementia Progression

--Clinical trajectories after a diagnosis of mild cognitive impairment (MCI) vary widely, so prognostic models must account for both the timing of subsequent diagnoses and unequal follow-up. Tau-PET subtype modifies the association between plasma p-tau217 and memory in Alzheimer's disease

INTRODUCTIONPlasma p-tau217 is associated with Alzheimers disease pathology and memory performance, but its implications for memory may depend on fibrillar tau topography.
